A PARENT OR LEGAL GUARDIAN MUST REGISTER A STUDENT.

You must have all the information listed below to register. We are sorry for any inconvenience this may cause; however, we are required to follow parish, state, and federal guidelines. If you do not have all of the information required, please take this form with you and return with all of the information.

_____ Birth Certificate

_____ Picture ID of Parent / Guardian (Custody Paper if applicable)

_____ Transfer / Withdrawal form after 1^st day of school

_____ Immunization / Health record

_____ Copy of last report card

_____ Copy of transcript

_____ Name, address, and phone number of last school if outside Jefferson Parish

_____ LEAP scores for all incoming 9^th graders

_____ GEE scores for all 10^th (if taken) 11^th, and 12^th graders

_____ Two current proofs of residence

                   1. Driver’s license or State ID

                   2. Water, Gas, electric, lease agreement, or anything from the State, Parish, or

                       Federal government.

                           - NOTE: Checks and bank statements are not considered proofs of residence.

                           - Both proofs of residence must be in the name of the parent/legal guardian 

                              registering  the student.

_____ A current evaluation and I.E.P. if the student is in Special Education.
